Care Team Supervisor

Samaritan Village is looking for an experienced professional to provide leadership, coordination, and support in the delivery of personal care services to our residents. This supervisory-level position functions as a liaison between residents, residents’ family members, physicians, and employees. As one of two Care Team Supervisors, and with the support of the Medication Management Supervisor and the Administrator, this role supervises all medication concerns, health issues and psycho-social needs of the residents and is the primary supervisor of the direct care staff in The Orchards.

We are looking for an individual with at least 2 years’ experience supervising in a senior living environment, ideally Assisted Living. Current RCFE Administrator Certification preferred. Samaritan Village has a team approach to providing care services to our residents, and this position will work closely with our management team, Care Team Supervisors and other department staff.  The ideal candidate will have a passion for working with seniors and their families.

Excellent communication skills, high ethical standards, a focus on collaboration and a firm understanding of quality customer service are all essential to the success of this position. The essential duties and functions of the position include, but are not limited to, the following:

Ensure all residents’ personal and medication documentation, medication distribution technique, physician orders, change of dosage, and other medication and health concerns are kept current and in compliance with all regulatory requirements.

With guidance from the Administrator: monitors, supervises, trains and assigns job duties for the direct care staff as well as evaluates job performance.
Interviews, assesses and evaluates prospective residents to be assured their health and personal needs can be met by Samaritan Village. Develops a written plan of care for each resident upon day of admission, updates quarterly and when resident’s physical or mental status changes.

Schedules, coordinates and participates in quarterly interdisciplinary plan of care meetings for each resident.
